Process Safety Incidents
For each of our PSM (process safety management) covered processes, we conduct regular process hazard analyses (PHAs) to identify possible release scenarios, review existing safeguards and verify that potential risks have been adequately addressed. We conduct a thorough root cause analysis using the TapRoot® methodology employed by agencies such as the U.S. Navy and NASA for incidents and near misses with high potential for injury or environmental impact. By identifying the root causes and developing corrective actions, we continue to take proactive measures to minimize risk.
A process safety incident is considered reportable to the ACC if it results in (a) a fire or explosion causing greater than $25,000 property damage; (b) a release of a chemical greater than the CERCLA reportable quantity for a hazardous substance or a release of 5,000 pounds; or (c) a serious injury or fatality arising from a fire, explosion or chemical release or release of energy or material from a process.
